I. Information must be provided for registration: Medical equipment suppliers are welcome to come to our hospital to bid (the official seal must be stamped on the information to prove its authenticity), and the specific contact with the equipment department.
1. Application conditions: up to two votes for domestic products (only for direct supply by the manufacturer or first-level agent) , and up to three votes for imported products (only for manufacturer, first-level agent or second-level agent) ;
2. The company's quotation and price commitment letter (contents to be reflected: product name, registration certificate name, registration certificate number, specification model, unit of measure, unit price, full name of the manufacturer, etc.);
3. Enterprises applying for medical device products must provide a copy of a valid medical device registration certificate (including the registration form);
4. Manufacturer qualification certificate and authorization letter;
5. Graded agent qualification certificate and authorization letter;

7. A copy of the legal person's and salesperson's ID card;
8. Credit information;
9. Graded after-sales service commitments and quality assurance agreements between manufacturers and agents;
10. Price comparison basis;
11. Import products must provide import certification materials from legal sources;
12. If the invested product is in the catalog of Fujian Province Medical Equipment Limited Price Sunshine Procurement Platform, screenshot printing is required;

Requirements for submission of information
1. The above qualification requirements must be stamped with an official seal. Each contract package must be individually and completely packed in a booklet in triplicate (one positive and two vices), and must be sealed with a document bag. Must indicate package number, bidding product name, bidding unit name, contact person, and telephone number;
2. The salesperson needs to bring his original ID card to present it on the spot to prove his identity;
3. The submitted project must provide product samples at the time of bid opening. If no sample is provided, the bid is invalid.
